2-Caregiver vs 3-Caregiver Rotation for 24-Hour Home Care in NJ
By Sofia Elmer, RN · Care Coordinator · 24 Hour Home Care NJ · 210 Haven Avenue, Suite 2C, Scotch Plains, NJ 07076
Two configurations cover almost every 24-hour home care case in New Jersey. Twelve hour shifts, usually 7 am to 7 pm and 7 pm to 7 am, define the 2-CHHA rotation. On three CHHAs, we split the day into eight hour blocks: 7 to 3, 3 to 11, and 11 to 7. Which one fits depends on acuity, sleep disturbance, and budget, not preference.
Call Sofia · (908) 912-6342How each rotation is built
Rotation shape is the math of coverage. Twenty-four hours of in-home support splits into two twelve hour shifts or three eight hour shifts, and we match the shift blocks to the household. According to 24 Hour Home Care NJ, the 7 am to 7 pm split is the most common 2-CHHA pattern in Union, Essex, and Morris counties because it mirrors a hospital nurse’s twelve hour pattern.
Three CHHAs on 7 to 3, 3 to 11, and 11 to 7 give every eight hour block a clear job. Morning hygiene, breakfast, physical therapy visits, and lunch sit with the 7 to 3 CHHA. Dinner, medication at 5 pm and 9 pm, and the sundowning window belong to the 3 to 11 CHHA. Overnight toileting, repositioning every two hours for pressure injury prevention, and any wake up events are handled by the 11 to 7 CHHA. For details on the money side, see 24hourhomecarenj.com’s 24-Hour Home Care in New Jersey · Full-Day Rotation page.
Both patterns use New Jersey CHHAs supervised by an RN under Board of Nursing oversight. CHHAs are W-2 employees, not independent contractors, which keeps the care plan enforceable. If you’re comparing a rotating 24-hour setup to a single live-in caregiver, our live-in caregivers NJ page walks through the fatigue tradeoffs.
The 2-CHHA case: continuity and lower cost
Two CHHAs on alternating twelve hour shifts means the client sees the same two faces, often for months. That matters for anyone with cognitive change, because familiarity reduces agitation. In an Essex County case running now, a 79 year old with early stage Alzheimer’s responds to her Monday Wednesday Friday CHHA by name and recognizes the Tuesday Thursday CHHA within ten seconds of greeting. Three faces, in her case, would have doubled orientation time at every shift start.
A 2-CHHA rotation also saves roughly $160 to $200 per day over 3-CHHA. One fewer caregiver per 24-hour window means one fewer eight hour wage block, one fewer travel reimbursement, and one less set of payroll taxes. On a 90 day post-hospital runway, that’s $14,000 to $18,000 over the quarter. Our NJ home care cost calculator at 24hourhomecarenj.com runs the full stack for your zip code.
What you give up: the back end of the night shift. A CHHA who starts at 7 pm and works until 7 am is doing light duty most of the time, but if the client wakes at 3 am and needs a toilet transfer, that CHHA is six hours into shift and still has four to go. Fatigue is real. According to 24 Hour Home Care NJ, the 2-CHHA pattern works best when the overnight client sleeps through, meaning two or fewer wake events between 11 pm and 6 am.

The 3-CHHA case: rest, FLSA, and middle of the night work
Three CHHAs on eight hour shifts solves the fatigue problem 2-CHHA can’t. The 11 pm to 7 am overnight CHHA starts rested, which matters when sundowning peaks at 2 am or a stroke patient needs bedside PT every three hours. In a Morris County post-stroke case, the overnight CHHA runs a leg range of motion sequence at midnight, 3 am, and 6 am to prevent contractures. A twelve hour overnight CHHA on hour eleven is not the clinician you want.
Our 3-CHHA rotation also keeps us cleanly inside federal wage and hour law. Fair Labor Standards Act (FLSA) rules count hours over 40 per week as overtime for CHHAs, and the 2013 Companionship Exemption revision made this stricter. Eight hour shifts, five days per CHHA, keeps every aide under the overtime trigger without games. U.S. Department of Labor Wage and Hour Division publishes the current direct care worker rules, which is the authority Sofia and the 24hourhomecarenj.com team follow.
Backup is also cleaner on three. If one of the three calls out Monday morning, the other two still have at least sixteen hours of rest between them, so one can extend a shift without a tired caregiver. On 2-CHHA, a callout means pulling a float CHHA on short notice, every time.
Handoff risk and information loss
Every shift change is a handoff, and every handoff is a chance for information to drop. A 2-CHHA rotation has one handoff per day. On three CHHAs we have two. That extra handoff is the real cost of the 8-hour pattern, and it’s not trivial for a dementia client whose intake pattern shifted at 4 pm but was observed by only the 3 to 11 CHHA.
Sofia closes the gap with a shift log book kept in the kitchen: three line entries at each handoff, time stamped. Fluid intake, bowel movements, falls or near falls, medication times, mood, sleep. The RN on the case reviews the log at every weekly home visit and flags trends. According to 24 Hour Home Care NJ, the clients where handoffs cause problems are the ones where the family skips the log and verbal reports replace written ones, every time.
Cost comparison for NJ 24-hour care
Here’s what each pattern costs in NJ as of 2026, before any long-term care insurance offset. Billing covers private pay and private insurance only, including John Hancock, Genworth, MetLife, Transamerica, and Mutual of Omaha, plus VA Aid and Attendance. Medicaid and Medicare are not accepted.
| Config | Shifts per day | Hours per CHHA | NJ day rate (2026) | Monthly (30 days) |
|---|---|---|---|---|
| 2-CHHA rotation | Two · 12 hr | 12 | $720 to $880 | $21,600 to $26,400 |
| 3-CHHA rotation | Three · 8 hr | 8 | $880 to $1,080 | $26,400 to $32,400 |
| Live-in (single caregiver) | One with sleep break | 24 (with 8 hr sleep) | $400 to $550 | $12,000 to $16,500 |
Live-in is on the table only when the client sleeps through, fewer than two wake events overnight, and when state live-in rules fit. For the single caregiver model, see our overnight home care in NJ page.
Which rotation fits which client
Low acuity companion cases, meaning stable chronic conditions, no overnight wandering, no PT at odd hours, and a budget sensitivity, fit the 2-CHHA pattern. We’ve run 2-CHHA rotations in Union County for post-hip-replacement clients, in Essex County for widowed clients, and in Somerset County for assisted-living to home transitions.
Higher acuity cases need the 3-CHHA pattern: dementia with sundowning after 4 pm, post-stroke within the first 90 days when PT and OT are heavy, hospice at home with symptom shifts, and clients on continuous oxygen or feeding tubes. According to 24 Hour Home Care NJ, every hospice at home case we’ve run in Morris County this year used 3-CHHA because end of life needs can’t wait for a tired overnight CHHA to warm up.
Our dementia home care NJ page covers why sundowning breaks the 2-CHHA pattern, and our post-stroke home care NJ page covers overnight PT cadence in the first 12 weeks after discharge.
How Sofia decides on the first intake call
Three questions settle the rotation choice. First: how many times does the client wake between 11 pm and 6 am on an average night? Zero to one wake events means 2-CHHA is viable. Two or more means 3-CHHA by default. Second: is there a diagnosed cognitive change with afternoon or evening agitation? If yes, 3-CHHA, no exceptions. Third: is there an active rehab plan with overnight exercises or a hospice symptom protocol? If yes, 3-CHHA.
I ask these in order on every intake. Answers map to a rotation in under ten minutes of conversation, and the family leaves with a specific day rate, not a range. Call Sofia at (908) 912-6342 and we’ll run the three questions. If you’d rather read first, start with home care after hospital discharge NJ, which is the discharge-day version of this decision.
FAQs
Can we start on 2-CHHA and move to 3-CHHA later?
Yes, and about one in four of our cases flexes this way. We start on 2-CHHA when overnight wake events are rare, then shift to 3-CHHA when sundowning or post-stroke needs intensify. The handoff is clean: we add the third CHHA to the roster, run a two day shadow, and the family sees one new face. Call (908) 912-6342 to plan the transition.
Does 2-CHHA ever hit overtime problems under FLSA?
Only if one of the two CHHAs covers more than 40 hours in a workweek, which happens when the second CHHA calls out and we don’t pull a float. We budget around this by maintaining a per diem float CHHA in every county we cover, including Union, Essex, Morris, Somerset, Middlesex, Bergen, and Monmouth. Overtime hits are flagged in our scheduling system before they clock.
What’s the actual sleep arrangement for the 11 pm to 7 am CHHA?
The overnight CHHA is awake and active for the full eight hours, not sleeping. This is different from a live-in caregiver, who gets an eight hour sleep break on the clock. If the client sleeps straight through, the overnight CHHA is doing light tasks, laundry, meal prep for breakfast, charting, and two hourly repositioning if the client is at pressure injury risk.
Do you accept Medicaid or Medicare for either rotation?
Billing covers private pay and private insurance only, including long-term care insurance from John Hancock, Genworth, MetLife, Transamerica, and Mutual of Omaha, plus VA Aid and Attendance for qualifying veterans and surviving spouses. Medicaid and Medicare are not accepted. If a family is trying to stretch a Medicaid waiver, we can usually suggest a lower acuity bridge until coverage lands.
How quickly can you start a 3-CHHA rotation after a hospital discharge?
Standard start time is 48 hours from the first phone call, and we’ve done 24 hours for Overlook, Morristown, and RWJ discharges. Three CHHAs plus an RN assessment is a bigger lift than two, so we confirm the full roster before the discharge day. Call (908) 912-6342 the day the discharge planner names a date, not the day of discharge.
