Overnight Only Home Care in NJ: When Is It Enough
Sofia Elmer, RN · Care Coordinator · 24 Hour Home Care NJ
A typical overnight shift in Union, Essex, and Morris Counties runs 10 pm to 8 am with one Certified Home Health Aide. We’ve placed overnight-only coverage for about 40 percent of our dementia-stage-two families since January 2026. The other 60 percent needed daytime hours added within 90 days.
Call Sofia · (908) 912-6342What a 10 pm to 8 am CHHA shift actually covers
Our standard overnight shift across Union, Essex, Somerset, and Morris Counties runs 10 pm to 8 am. One Certified Home Health Aide, one house, one parent. The aide arrives 15 minutes early for a handoff from the family, reviews the care-plan binder, and does a safety sweep before bedtime.
According to 24 Hour Home Care NJ, 92 percent of our overnight placements since January 2026 have been single-aide shifts. The remaining 8 percent used a two-aide rotation because a spouse with early-stage dementia also lived in the home. Call (908) 912-6342 and Sofia will ask the four questions that decide which model applies.
Our aides stay awake. We don’t send sleep-shift aides and we won’t quote that model. NJ families who tried sleep-shift coverage with other agencies often tell us they lost a parent to a 2 am bathroom fall because the aide was asleep two rooms away. Overnight CHHAs from 24hourhomecarenj.com sit near the bedroom with a soft light, a book, and the care binder.
The six overnight tasks we document every night
Every overnight shift closes with a written log entry sent to the family contact by 8:30 am. Six documented tasks appear on every log: safety sweep, bedtime routine, medication, overnight bathroom escorts, hydration check, and morning handoff. Each carries a timestamp.
According to 24 Hour Home Care NJ, the safety sweep takes about 12 minutes. Aides confirm the stove is off, lock the front and back doors, place a nightlight in the hallway, move slippers to the bedside, and clear any throw rugs that have shifted. Eleven of our 2026 overnight clients had at least one throw rug we removed on week one; families should bag them in a closet before coverage starts.
Bedtime routine runs 9 pm to 10:15 pm depending on family preference. CHHAs help with toilet, teeth, and pajamas, lay out morning clothes, pour a bedside water glass, and read the medication list aloud. Medications between 10 pm and 7 am are logged with time, name, and dose. Should a parent need insulin or a late-night blood thinner injection, we loop in a visiting RN since CHHAs aren’t licensed to administer injections in New Jersey.
Not sure if overnight only is right?
Sofia will walk through your parent’s nighttime routine on a 20-minute call and tell you honestly whether overnight only will hold, or whether daytime hours are needed too.
Call (908) 912-6342When overnight only is enough
Overnight-only coverage fits when a parent is independent during daylight hours and the family’s worry starts at sundown. Four markers tell us the parent fits this profile: 0 to 2 nighttime wake-ups per night on average documented over the last two weeks, no fall in the last 90 days, no diagnosed sundowning or wandering, and a daytime support system (spouse, adult child, or senior center program) that covers 8 am to 10 pm.
We’re seeing more of this pattern in Westfield, Summit, and Chatham since Overlook Medical Center discharge-planning teams started referring post-hospital patients to us for a 30-day overnight bridge. A patient recovers during the day with family support, and the overnight CHHA covers the risky hours. See the post-hospital discharge care in NJ page for the 72-hour admission protocol.
Loneliness is a legitimate reason to add overnight coverage even when there’s no clinical risk. Eighteen percent of our 2026 overnight starts list “spouse recently passed” or “adult child moved out of state” as the primary reason. A steady overnight CHHA (same aide, three to five nights per week) is a workable answer.
Five signals that overnight only won’t hold
Here are the signals that overnight-only coverage won’t be enough, and 24-hour rotation becomes the safer plan. Any one in isolation is a reason to call us for a longer assessment. Two or more and we’ll recommend 24-hour from day one.
- More than 2 nighttime wake-ups on average. A parent up three or more times racks up daytime energy debt and shows daytime confusion. That’s a dementia-progression warning, not a sleep problem.
- Overnight sundowning. Agitation, pacing, or repeated requests to leave the house after 7 pm. See the dementia home care in NJ page for the sundowning protocol.
- Wandering risk. Any attempt to leave the house at night in the last 90 days, or a diagnosed wandering history on the Alzheimer’s Association safety assessment. A single aide can’t block the front door while helping with the toilet in the master bedroom.
- Falls in the last 90 days. According to the National Institute on Aging (NIA falls prevention), one in four adults 65-plus falls each year, and the risk doubles after a first fall. Daytime supervision becomes essential.
- Medication complexity above six prescriptions. When timing crosses the day/night boundary (a 9 pm dose and a 7 am dose), someone qualified has to be present at both. If neither family nor aide can reliably cover one side, 24-hour rotation closes the gap.
The cost math: $300 overnight vs $720 to $1,080 for 24-hour
Overnight only at $30 per hour across 10 hours works out to about $300 per night, or $2,100 per week for seven nights. Our 24-hour rotation (two or three CHHAs rotating through the day) runs $720 to $1,080 per day in 2026, or $5,040 to $7,560 per week. That’s roughly $3,000 to $5,500 per week in difference.
Live-in is a third option. A live-in CHHA stays in the home for 24 hours per shift with a defined 8-hour sleep period and is priced $400 to $550 per day. See the live-in caregivers in NJ page for staffing rules. Live-in is cheaper than 24-hour rotation but requires a bedroom for the aide and isn’t appropriate when nighttime wake-ups exceed two per night.
| Coverage model | Hours | 2026 rate | Weekly cost | Best when |
|---|---|---|---|---|
| Overnight only | 10 pm to 8 am (10 hr) | $30 per hour | $2,100 | Daytime stable, 0 to 2 wake-ups, no 90-day fall |
| Live-in | 24 hr with 8-hr sleep | $400 to $550 per day | $2,800 to $3,850 | Private bedroom available, under 2 wake-ups |
| 24-hour rotation | Two or three CHHAs | $720 to $1,080 per day | $5,040 to $7,560 | Dementia, wandering, falls, sundowning |

How to start overnight coverage in NJ
Starting coverage takes 48 to 72 hours from the first call. On day one, Sofia or a Care Coordinator does the 20-minute phone intake and we send the Service Agreement by email. Our clinical team (an RN) completes an in-home assessment of the bedroom, bathroom, and medication list on day two. By day three, the first CHHA arrives for the 10 pm shift. Families should plan to be present the first night for a one-hour overlap, 9:30 pm to 10:30 pm.
For deeper coverage areas, see Union County NJ home care and Essex County NJ home care. We also serve Morris County NJ home care with same-week start dates. For the overnight model specifically, the companion piece is overnight home care in NJ.
Families already weighing 24-hour coverage because of dementia or a recent hospital stay should start on our 24-Hour Home Care in New Jersey · Overnight Caregivers page. It covers the full staffing model, aide credentials, and first-week protocol. If overnight-only fits better, Sofia will say so on the intake call.
Overnight only home care in NJ: FAQs
How many hours is a standard overnight shift?
Our standard overnight shift runs 10 pm to 8 am, which is 10 hours. We can extend to 9 pm start or 9 am end on request, bringing the shift to 11 or 12 hours. Shorter shifts (8 hours or less) are available but we don’t recommend them because the aide-fatigue curve starts affecting documentation quality. Call (908) 912-6342 to design the shift window.
Does the overnight CHHA sleep?
No. Our overnight CHHAs stay awake the entire shift. They sit in a chair near the bedroom with soft lighting and the care binder, do hourly safety checks, and document every bathroom escort and medication. If you want a sleep-shift model, that’s a live-in arrangement with a protected 8-hour sleep period and a private bedroom for the aide.
What’s the earliest you can start overnight coverage?
Our fastest start since January 2026 was 11 hours from first call to first overnight shift, for an Essex County post-stroke discharge. Typical start is 48 to 72 hours because we want the clinical assessment and medication reconciliation done first. Call (908) 912-6342 if a hospital discharge planner is pushing for same-day coverage.
Can we switch from overnight only to 24-hour later?
Yes, and roughly 60 percent of our dementia-stage-two overnight clients do within 90 days. We don’t charge a transition fee. The RN who did the initial assessment returns for a 15-minute re-assessment, we update the Service Agreement, and we add the daytime CHHAs the following week. No notice period required.
Do you accept Medicaid or Medicare for overnight care?
No. We are private pay and private insurance only. We do not accept Medicaid or Medicare. Long-term care insurance from John Hancock, Genworth, MetLife, Transamerica, and Mutual of Omaha reimburses most overnight hours after the elimination period. VA Aid and Attendance covers eligible veterans up to $2,795 per month in 2026. For Medicaid-funded coverage, NJ 211 (nj211.org) can connect you to Medicaid Managed Long-Term Services.
