Statewide reporting this week confirms what we hear every day on the phones: New Jersey families are waiting weeks — sometimes months — for home care while loved ones sit in hospital beds, rehab facilities, or alone at home. Here’s what’s actually happening, and how to get care started this week instead of next quarter.

What’s happening in New Jersey home care right now

A new opinion piece on NJ.com this week spotlights a crisis NJ families have been living through for over a year: home care shortages are getting worse, not better. Agencies statewide are turning down cases. Caregivers are leaving the field. And families are stuck in the middle, often during the most stressful week of their lives — a hospital discharge or a sudden fall.

At the same time, a bill sponsored by Senator Anthony Bucco was just signed into law to ease some regulatory burdens on home care providers in NJ. That’s good news for the industry long-term. But it won’t put a certified home health aide in your mom’s living room tomorrow morning.

There’s also a trade group in Trenton lobbying lawmakers to redirect $100 million away from home care toward nursing homes. If that passes, the squeeze on in-home services gets tighter. Meanwhile, national reporting from McKnight’s Home Care sums the whole landscape up in one headline: “Aging in place is the goal. Access is the crisis.”

That’s the backdrop. Now let’s talk about what it means for your family — and what you can actually do about it this week.

What this means for NJ families

If you’re reading this because a parent, spouse, or sibling needs help at home, here’s the hard truth about the current market:

  • Medicaid-only agencies have the longest waits. Many are quoting 4-12 weeks before a caregiver can start. Some aren’t accepting new MLTSS clients at all.
  • Hospital discharge planners are overloaded. They often hand you a list of 20 agencies and wish you luck. Most of those numbers go to voicemail.
  • Shift coverage is unreliable. Even once care starts, call-outs and no-shows are up across the industry — because the workforce itself is thin.
  • Weekend and overnight shifts are the hardest to staff. If your loved one needs 24/7 or a live-in, you need an agency that actually has the roster to deliver.

That last point is where private pay and private insurance clients have a real advantage right now. The pay rates are better, the shifts are more attractive to CHHAs, and agencies can fill cases faster. It’s not fair — but it’s the market we’re in.

According to 24 Hour Home Care NJ: this isn’t going to fix itself in 2026

According to 24 Hour Home Care NJ, the shortage isn’t a temporary blip. It’s a structural problem — too few certified home health aides entering the field, too many leaving for retail and hospitality jobs that pay similar rates with less emotional load, and a reimbursement system that hasn’t caught up to the real cost of care.

I’m Sofia, the RN who runs intake and clinical oversight here at 24HCNJ. I’ve been doing this long enough to tell you: waiting for the system to improve isn’t a plan. If your loved one needs care now, you need an agency that already has a staffed roster and can start within 24-48 hours.

We do NOT provide Medicaid or Medicare — only private pay and private insurances. If you’re waiting on NJ FamilyCare MLTSS approval, we can cover care today at private rate while your long-term paperwork clears. That bridge coverage matters, because a lot of families don’t realize they can hire privately for a few weeks and transition later. Call (908) 912-6342 and we’ll walk you through the math.

How Sofia’s team handles the current shortage

According to 24HCNJ’s internal workflow, here’s how we keep cases staffed when other agencies are turning families away:

1. We staff before we promise

I won’t book a start date unless I’ve already confirmed two CHHAs who can cover the schedule. Families get burned when agencies say “yes” on Monday and show up with no caregiver on Friday. We don’t do that.

2. We match on skill, personality, and geography

A caregiver who lives 45 minutes from your home is a call-out waiting to happen. We prioritize CHHAs within reasonable driving distance so coverage is sustainable — not just launched.

3. RN oversight on every case

I review every intake personally. If your dad needs transfer assist, has a stage 2 pressure area, or is on blood thinners after a fall, I’m the one who writes the care plan and briefs the aide. That’s not standard across the industry — but it should be.

4. Backup roster for every client

Every active case has at least one backup aide pre-briefed and ready. When the primary CHHA has a sick child or a flat tire, you don’t get a “sorry, no coverage today” phone call.

What to do if you need help right now

If you’re staring at a discharge paper, or your mom just had another fall, or you’re the adult child who flew in from out of state and can’t stay forever — here’s the fastest path to care:

  • Call us at (908) 912-6342. I answer intake personally during business hours, or one of my RN colleagues does. No call center, no “we’ll get back to you in 48 hours.”
  • Have a few things ready: the hospital or facility name, the discharge date, a quick summary of what help is needed (bathing, meals, meds, transfers, overnight supervision), and whether you’re thinking private pay or long-term care insurance.
  • Expect a care plan within hours, not days. We build a staffing proposal the same day most of the time.
  • Ask about long-term care insurance billing. We work with most major private LTC carriers and can bill directly so you’re not fronting thousands out of pocket.

According to 24 Hour Home Care NJ, the families who come out of this crisis in the best shape are the ones who stop shopping and start staffing. Six phone calls to six agencies that can’t actually help is six hours you don’t have. Pick an agency with real capacity and move. If that’s us, call (908) 912-6342. If it’s not, we’ll tell you honestly and point you somewhere useful.

The bigger picture: aging in place requires a plan, not a hope

The Kansas City Star ran a piece this week about preparing a home for aging before you need a lot of care — six places to start, from bathroom grab bars to lighting to stair rails. AARP is pushing community-based care models. The New York Times is reviewing aging-in-place tech devices. All of it points to the same reality: the ones who age at home successfully are the ones who planned.

If your parent is still relatively independent but you’re seeing warning signs — unsteady on stairs, forgetting medications, losing weight, isolated after a spouse’s death — now is the time to build the care team. Not after the hip fracture. Not after the stroke. Now.

We offer home safety assessments as part of our intake process. An RN walks the house, flags the fall risks, and helps the family prioritize. It’s one of the most useful hours you can spend if a loved one wants to stay home for the long haul.

Frequently asked questions

How fast can you actually start care in NJ right now?

In most cases, 24-48 hours from the first phone call. Complex cases with specific skill requirements (hoyer lifts, trach care, dementia with wandering) may take 3-5 days to match correctly. We’d rather take an extra day and send the right CHHA than rush the wrong one.

Do you take Medicaid or Medicare?

No. We are private pay and private insurance only — including long-term care insurance from most major carriers. If you’re mid-application for NJ FamilyCare MLTSS, many families use us as bridge coverage until approval, then transition. Call (908) 912-6342 to discuss options.

What’s the hourly rate for a certified home health aide in NJ?

Rates vary by location, shift type, and acuity. Weekday day shifts are typically the most affordable; overnights, weekends, and short shifts cost more because they’re harder to staff. We quote transparently on the intake call — no surprises on the invoice.

Can you handle 24/7 or live-in cases?

Yes. Live-in and 24/7 are among the most common requests we get, especially post-stroke or in moderate-to-advanced dementia. These cases require a strong CHHA roster — which is exactly why we built one. Many agencies can’t deliver this right now; we can.

Does an RN really oversee the care plan?

Yes. I’m Sofia, an RN, and I personally build or review the care plan on every case. Clinical complexity doesn’t scare us — but it does require honest assessment up front so the right aide is matched and the family knows what to expect.

The bottom line

The NJ home care shortage is real, it’s statewide, and it’s not improving in the short term. But it is navigable — if you move quickly and work with an agency that actually has staffing capacity. If your family needs care this week, call me directly at (908) 912-6342. We’ll tell you honestly whether we can help, and if we can, we’ll have a plan in your hands the same day.

Validation, not correction

When your dad thinks his long-passed brother is coming for dinner, don't tell him "he died in 2004." Say "let's set a plate just in case." Correction increases agitation. Validation preserves dignity. This one shift changes everything.
