How our caregivers support wound care

Certified Home Health Aides in New Jersey do not perform sterile wound care — that stays with the licensed home-health nurse. But our caregivers can:

  • Support the sterile technique environment. Wash hands, sanitize the workspace, hand supplies to the family or nurse during the change. Position the client for wound access.
  • Daily wound observation. Notice changes in size, color, exudate, odor. Report to Sofia within the hour. Sofia calls the wound-care nurse or family accordingly.
  • Hygiene discipline around the wound. Redirecting shower spray away from the dressing. Managing incontinence to protect a sacral wound. Keeping the wound area dry.
  • Nutrition support for healing. Protein-forward meals. Hydration discipline. Zinc-rich foods. High-vitamin-C fruits.
  • Positioning. Turning every 2 hours to prevent additional pressure sores. Specific pillows and positioning technique.
  • Pain observation. Recognizing pain patterns that suggest infection vs normal healing pain. Reporting to Sofia.
  • Ostomy support (colostomy, ileostomy, urostomy). Emptying and rinsing the pouch. Skin protection around the stoma. Recognizing signs of stoma prolapse or retraction.

Coordination with your wound-care nurse

Most NJ wound-care cases involve a home-health nursing agency (Bayada, VNA, Interim, Family First, Amedisys) visiting 2-3 times per week for sterile dressing changes. That agency is who bills Medicare for the skilled care.

Our role is different: we cover the daily in-home presence in the hours between nurse visits. We report to the wound-care nurse, not the other way around. When the nurse visits Tuesday and says “the wound looks like it’s beginning to show early infection signs,” Sofia has already known that on Monday because our caregiver observed and reported it Sunday night.

Can your caregivers change my mother’s wound dressing?

No. Sterile dressing changes require a licensed nurse (LPN or RN). Our Certified Home Health Aides support the sterile-technique environment — hand supplies, position the client, hygiene discipline — but the actual dressing change stays with the licensed clinician.

Then what value do you add?

Daily observation between nurse visits. If the nurse comes Monday and Thursday, we’re the eyes on the wound Tuesday, Wednesday, Friday, Saturday, Sunday. Sofia reports observations to the wound-care nurse within the hour. Early infection catches happen this way.

How does this work with my parent’s home-health nurse?

Complementary. Home-health nursing (Bayada, VNA, Interim, etc.) is Medicare-covered short-term skilled care. Our service is private-pay continuous personal care. Sofia coordinates directly with the home-health nurse’s care plan.

Can my parent’s LTC Insurance cover wound-care home care?

Yes for the personal-care portion. LTCi typically covers our services under the standard daily benefit. Medicare covers the skilled nursing separately. Sofia files LTCi at no charge.

Do you handle ostomy care?

Yes. Our caregivers support pouch emptying, rinsing, skin protection around the stoma, and observation for stoma complications. The initial ostomy education comes from the WOCN (Wound Ostomy Continence Nurse) — we then support the daily rhythm.
