Post-hospital-discharge crisis. Case manager at Overlook, Trinitas, Robert Wood Johnson Rahway, or JFK University Medical Center is pushing a discharge tomorrow or today. The plan says “with family support.” Family cannot provide that support tonight. The client cannot be alone.

Post-fall or sudden decline. Parent fell in the kitchen this morning. ER cleared them physically, but they need eyes-on tonight. Adult child is out of state or works nights. The alternative is a hospital admission that everyone knows would be worse than staying home.

Family-caregiver collapse. The daughter or son who has been the primary caregiver for 6 months just told you they cannot do tonight. Not can-not-tolerate — cannot physically. Marriage is fraying. Kids are noticing. The system needs another person in it starting today.

Sibling flight-in with hard exit. Sister is flying in from California, staying 48 hours, flies back Wednesday. There has to be a caregiver in place when she leaves or mom cannot stay home.

The 8-step arrival checklist

Here is what happens between the phone call and the caregiver walking through the door. According to 24 Hour Home Care NJ, this is the standard operating protocol for a same-day Union County start:
