Why hospital discharge coordination is different
Discharge is not a scheduled event — it is a compression window that starts the moment the attending physician writes the discharge order, and ends when the client is safely at home with a caregiver present. What happens in that window determines whether the recovery arc succeeds or the client is back in the ER within 30 days.
The 30-day readmission risk. Medicare tracks 30-day hospital readmission rates aggressively. For most conditions (heart failure, pneumonia, hip fracture, stroke), the 30-day readmission rate is 15-25 percent when discharge coordination is weak. Do you accept Medicare for post-discharge home care?
