Why hospital discharge coordination is different

Discharge is not a scheduled event — it is a compression window that starts the moment the attending physician writes the discharge order, and ends when the client is safely at home with a caregiver present. What happens in that window determines whether the recovery arc succeeds or the client is back in the ER within 30 days.

The 30-day readmission risk. Medicare tracks 30-day hospital readmission rates aggressively. For most conditions (heart failure, pneumonia, hip fracture, stroke), the 30-day readmission rate is 15-25 percent when discharge coordination is weak. What if the hospital case manager tells us to use their contracted home care provider?

Hospitals have contracted providers for Medicare-covered short-term skilled home health (nursing, PT, OT). That’s a different service from continuous personal care. Sofia’s caregivers complement the Medicare-covered nursing — you’re not choosing between them.

Can Sofia coordinate the DME delivery?

Yes. Sofia coordinates DME delivery timing with the discharge planner and the DME vendor (typically Rotech, Lincare, Apria, or hospital-affiliated DME service). The goal is DME arrives at home before or with the caregiver.

Do you accept Medicare for post-discharge home care?
