Step 2: Free RN Assessment
Our registered nurse visits your loved one’s home to conduct a comprehensive assessment — at no cost and no obligation. The RN evaluates:
- Medical conditions and medications
- Cognitive function and behavioral patterns
- Mobility, fall risk, and home safety
- Nutritional needs and dietary restrictions
- Daily routines and personal preferences
- Family dynamics and caregiver involvement
Step 3: Personalized Care Plan
Based on the assessment, our RN designs a detailed care plan covering:
- Recommended service level — companion, live-in, overnight, or 24-hour care
- Specific ADLs and health monitoring tasks
- Medication schedules and dietary needs
- Emergency protocols and physician contact information
- Caregiver matching criteria — personality, language, skills
Step 4: Caregiver Matching
We select a caregiver (or team) based on your loved one’s care plan, personality, language, cultural background, and scheduling needs. You can meet the caregiver before care begins — and if the match isn’t right, we’ll reassign promptly.
Step 5: Care Begins
Your caregiver arrives and starts following the care plan. During the first week, our RN checks in frequently to ensure a smooth transition and adjust anything that needs fine-tuning.
