Caregiver Match Criteria

We learn about your loved one’s personality, interests, daily routine, communication style, and family preferences. This allows us to identify caregivers who are not just qualified — but genuinely well-suited to the individual.

Written Care Plan

Before leaving, our RN documents everything in a written care plan that you receive a copy of. This becomes the blueprint for your caregiver’s daily responsibilities and is updated at every RN supervisory visit going forward.

A licensed registered nurse arrives at the scheduled time. The assessment typically takes 45 to 75 minutes. Family members are encouraged to be present — the more context we have, the better the care plan we can develop.

Before leaving, the RN reviews her findings with your family and presents a written care plan with specific recommendations. You are under no obligation to proceed with our services.

What to Prepare Before the Assessment

To make the most of your free assessment, it helps to have the following available — though nothing is strictly required:

  • A current list of medications (prescription and over-the-counter)
  • Recent medical records, discharge summaries, or specialist notes if available
  • Names and contact information for your loved one’s physicians
  • Any previous home care or rehabilitation notes
